The Tri Hita Karana Project is a proposed province-wide, AI-first precision health ecosystem for Bali—joining clinical care, citizen Digital Twins, public-health navigation, cultural integrity, interoperability and measurable community benefit.
THE REFRAME
A health system does not operate inside a culture. It is part of the culture—and must earn the right to serve it.
The project takes its name from the Balinese philosophy of harmony among people, nature and the spiritual. Technology is positioned as an enabling layer within that relationship, not a force that erases it.

The proposed program begins with local implementation contexts and expands through stage-gated governance, local leadership and independent evidence.
Citizen layer
A Bali Health Companion, Shae Digital Twin, family accounts, low-bandwidth access and navigation through public services.
Clinical layer
CLIA support for hospitals, clinics and puskesmas, with clinician training and human-in-the-loop decision support.
Interoperability
RME and SATUSEHAT FHIR integration, consent infrastructure, identity harmonisation and auditable data flows.
Cultural integration
Ethically credentialed traditional pathways, Wariga-related cultural content and oversight through cultural authorities.
Community value
Health points, navigator workforces, village benefits and programs designed to increase screening and prevention.
Evidence & governance
Clinical safety, health data trust, privacy-by-design, independent evaluation and transparent outcome reporting.

From a flagship centre to a province-wide system
A proposed Besakih Holistic Health Centre serves as a visible node for diagnostics, recovery, training, research and culturally respectful integrative health. It is one enabling asset inside a much wider public-health transformation.
The project remains subject to partnership agreements, local law, clinical validation, data governance, community consent, funding diligence and regulator engagement.
The most advanced health system is not the one with the most technology. It is the one whose technology remains in right relationship with people and place.

A replicable model—only if it remains authentically Balinese.
